The game is called TERA. It already exists, and EQ's lore was hamfisted on a good day, which is completely unlike anything in the Souls' universe(s).
EQ could probably run on a smart watch, since it is literally almost 20 years old at this point. The issue is connectivity, and since we aren't Asia it's a bit of a mess to have wireless connection 100% of the time in the states. Having EQ on a DS is literally just begging for the "oh shit, sorry got disconnected" as the group recovers as a standard.
