I've worked in a chemical plant that is over 150 years old so it has a big history of death occuring. The latest was in 2011, when someone lost a pen in a tank, and wanted to get it back. The tank was full of nitrogen, so he dons his gas mask and go down in it..
Unfortunately, a gas mask does not create oxygen, so he died at the bottom of the tank.
GIS for gas mask. Looks pretty safe
