Because it's rare for people to be happy with just PVP being enabled. They want balancing and content and shit like that all takes development resources.
I doubt anyone would have a problem if they simply enabled players to attack each other and did nothing else.
That was one thing I hated about EQ (and other games) balancing. Shit getting nerfed across the board because it was OP in PVP. They’d fucking ruin classes/abilities in PVE doing that shit. I always wished they would have had the spell just do different things depending on if it was targeted at a PC or NPC. Too hard to code I guess? Idk.
