That chart is incredibly misleading, they are including the per-capita cost of Medicaid (which you lose if you get a job) in those numbers for each state. Health care has become so expensive that in some states it now costs Medicaid over $20,000 a year to provide care for a family of four. So yeah, it's pretty easy to get >$15/hour in benefits if you're starting with $20k in "income" for health insurance.
Remember that if you have a $20/hour job, it probably costs your employer close to $40/hour to employ you. He has to pay his share of the OASDI taxes, he has to pay for unemployment insurance and workers comp, and he has to pay for your health care, because very few $20+/hour jobs come without a group plan.
