Diabetes seems like a reasonable bet, especially if it is poorly controlled. Amputation due to peripheral artery disease/nerve damage, or getting some type of infection due to foot ulcerations, or (I forget the name of the condition) your bones become super fucking soft and even walking can cause fractures, can happen. His other leg looking very discolored and swollen is concerning.
