yea, at least you can admit to being a fuckface, that's a step in the right direction. Now if you can find the willpower to simply ignore the others and focus on your game you might see the results you hope for. I traveled down your path a long time ago and it leads nowhere good. I might have the brief lapse of judgement from time-to-time but for the most part I don't rage anymore.
Basically when I first got to gold it was when I finally adopted the attitude that "everyone else is terrible, I'll have to carry". If I lose a game I simply couldn't carry. Nowadays I have relaxed a bit on that attitude and I try to be a team player, but being a team player has kinda kept me in limbo hovering in low gold. I pick team oriented champs like Syndra mid, whom I've played ~200 games with and only have a 50.7% win rate. I've lost tons of syndra games where I had 15+ kills yet due to being on a team oriented champ, I couldn't use the gold to carry hard enough.
Ideally you need confidence in your abilities and you need to play champs that can carry such as:
Top lane: Tryndamere/Riven/Jax
Mid: Yasuo/Katarina/Akali
Jungle: Kha Zix/Pantheon/Udyr(cough)
ADC: Trist/Lucian
Support: Morgana/Leona/Thresh
My advice is cut the attitude fast, at least get out your rage before season 5 and start learning ways to carry yourself.
You will always have bad games, learn to avoid raging back when teammates rage at you. Take your loss and move on fresh.

Losses will happen and can be devastating for morale. Keep calm, move on fresh, the best advice is if you lose 2-3 in a row, walk away and try again later.
I've also found that if I win a couple and walk away I tend to develop much longer win streaks. I've had days where I played 10-15 games in a row and I'd be lucky to be even at the end, then I'd have streaks of 15+ wins by just logging in for 1-2 games, winning, and stopping until the next day.
