Near as anyone has been able to tell, Gaz doesn't have a QA team. They rely entirely on the testing done by players on the test center to QA new stuff. Afaik nobody has ever seen Gaz advertise for QA positions. It's possible that they use a temp service to provide warm bodies for QA, or they may outsource their QA, but it seems most likely that they just don't have a QA department at all. Devs make sure that the basic functionality is intact in a new build, then they push it to test center and let the players do the QA for free.It's clear their internal testing team is beyond retarded as every new event they come out with is basically worth doing for about an hour before you get tired of it. I'd say the main reason behind a lot of it is all the people that are now at or approaching omega cap, its hard to find motivation to grind for a currency to purchase items that are usually worse then hero specific uniques and the boss artifacts.
The one video I watched from Limbo was basically a full group in uniques and shit and then all of them were using the DPS test medal from TC which gives you the equivalent of a full set of gear(crappy gear) on one slot. So basically, everyone had like twice the stats they'd have overall, and surprise, they were clearing stuff in literally seconds. If that's what they used to determine balance, then yeah it's fucking problematic.I mean, they also do stupid as hell shit like making changes to Limbo then pushing it live without testing those changes. Or failing to take into account that the vast majority of their TC players are people with 95% BIS or better, which far exceeds the majority of the playerbase and might account for why Limbo seemed fine on test but was pretty nightmarish when it went live.
I had this conversation with a friend in game. His point of view is that Marvel Heroes is a game where you get to play your favorite hero from the comics and the focus isnt on being able to play unique playstyles. Thats why every hero has been getting boiled down to the same kit. 1-3 DoTs/Damage layers, One trigger skill to make your spender do more damage (a skill on cooldown, effected by this skill, boss, etc), and your primary spender that does % more damage against enemies when --They posted this already:

Players entering incorrect versions of Limbo - I.E. half finished. Seems to be an older issue with the queue but we're seeing what we can find/fix.
Bosses occasionally not spawning.
Fucking patcher is slow as all fuck though for me. Also I still have trouble deciding what I want to play, there's still this weird "same-y" feeling from every hero, it feels like a lot of stuff plays more or less the same and it's kinda annoying. Maybe I'll just pick up my Scarlet Witch to see some of the new content until I figure out something better. Seems kinda too far behind to do any of this new limbo nonsense though.
It's being tuned down. You should have no problem pugging it tomorrow, despite the fact that you're in a SG constantly advertising room in limbo groups and having no trouble completing the fights.I don't know if it is or not but Limbo definitely needs to be behind the cosmic trial wall.
