My EQ mage on sullon zek. Played with the random name generator for about 20 minutes till I had something similar to it (antalious or something maybe) and then I just customized it and it seemed to be kinda roman sounding and also similar to antonius from EQ lore. Also been known as DeadlyJustice for years in QuakeWorld and X-wing Vs Tie Fighter and 6 or so years in WoW as lolpaladins.

EQ name for an Iksar female. It was meant to be Sylia, but at the time there was no support for non-Qwerty keyboard so it accidentally became Szlia (z and y are switched on swiss keyboards along with punctuation being in different places). When I noticed the mistake after a couple of hours, I said "Damn! I already spentall this timeplaying, I am not starting over for a typo!" In the end it was for the best I think, it's more memorable and lizardish than Sylia.

When I played D2 I had a bitch of a time finding a name that was allowed, they all seemed to be taken. I said "fuck it" and just typed in Noodleface out of anger. I then played Noodleface the bowazon for glory for months. I didn't use it for awhile, but now it's the name I use in every game.
I was also Aria on fohguild's forum. That name came from my FFXI character name. I jokingly named my char Ariagiovanni after the soft-core pornstar, with no intention on keeping it. I played her for like 6 years straight. I always hated the name after I made it, but since you invested so much time in your character in that game, that was it.
Occasionally I will use the name Kishin, something about a japanese Demon.

Vatoreus comes from my old MUD days, and is based off RA Salvatore. Malfost was another name I came up for at around the same time. Cymric is another, but don't really use it as much.
Started playing Anarchy Online about 8 years ago, all the names i wanted were either taken or to long, got frustrated and hit the random button, justonce, because I'm hard core like that... stopped playing for a while and sold my account but still use the name for everything. Ended up adding the 42 since it is the answer to life, the universe, and everything in it.
