Ingame maps are fine. I think how WoW did them was perfect (Have them blank at first and unveil as you progress). You could even combine them with EQ's system with player-made notes and add a structure to share them. Personally with EQ I just downloaded a map pack and put it in the right place, which is probably what would happen.
Open world PVP probably won't be a core feature of Pantheon. I don't really like open world PVP unless the game is built around it.
I think instancing should be limited to only raid zones. If a zone is too crowded you should move. If all the zones for your level are too crowded, oh well. It's still better than instancing for everything.
I don't think people are clamoring for long grinds and hell levels. I actually think a lot of people are starting to want to move away from levels somewhat. Draegan and I post the same idea frequently where there are no levels and you roam around the world doing shit that improves you. I think Aradune has posted about having levels + having achievements that give stats.
One thing I'd love to see tried in an MMO is to completely remove exp based levels and to have an NPC run faction that has some testing grounds that give you a rating based on a solo performance. Your skill level, gear, and misc stats will determine your ability to score highly in the testing grounds. Group-based roles like healer could be tested in a very similar way to the benediction quest in WoW:Quest:The Balance of Light and Shadow - WoWWiki - Your guide to the World of Warcraft
The level would be just a number that player's can see and maybe some item-based rewards or modest stat gain from improving your level. I think it'd be rife with exploits, test-specific skills and not be a perfect estimation of someone's quality but it'd be fun and would separate the coattail riders with tons of free time from the pro-gamers. It was always funny watching the scrub priests need to beg for someone to log onto their account to do the benediction quest.
